Everything You Need to Join Our Dental Family
Our Dentist and team members are honored that you’re considering Deerfield Valley Dental Care for all your dental needs. As you prepare for your appointment, we do recommend that you take a look at our new patient information page below so you can be better prepared. This includes expectations to maintain for your first visit, patient forms you’re able to fill out, and information on financing. You’re also welcome to give us a call if you have any concerns or questions you need answered.
Your First Visit
Our practice is committed to providing you and your family with safe, gentle, and high-quality dental care. We understand that you or your child may feel anxious about visiting the dentist. We are sensitive to your needs, and it is our goal to make you feel comfortable visiting our practice while providing the best care possible.
During your first visit, we make sure to obtain important background information, such as your dental and medical history, take X-rays of your teeth and mouth. Then our Dentist will meet with you to discuss your X-rays, help you create your own individual treatment plan, and answer any questions you may have.
At your next visit, a hygienist will clean and polish your teeth, and inspect your teeth and gums for cavities or early signs of tooth decay or gingivitis.
We understand you have a busy schedule, so when you’re preparing to visit our office, please remember your appointments can last between 45 and 90 minutes.
We look forward to your first visit.
Patient Forms
New patient forms allow our Dentist to learn more about your medical history and any medications you are currently taking that could influence your future treatments. This makes it easier for our team to personalize your care and understand your oral health needs. For convenience, you can click on the New Patient Packet link to complete the digital forms prior to your appointment.
New Patient Packet Annual Patient Update (Medical Info & HIPAA)
Unrestricted Insurance Provider
What that means for you is we collect for the appointment at time of service, we submit the claim to your insurance company and your insurance company will reimburse you the amount that they cover for your appointment. The insurance will send the reimbursement directly to you.
Financing
There are viable alternatives for financing your dental care expenses, such as Cherry Financing and CareCredit. These third-party financing options provide a practical solution. With minimal or no interest applied to your monthly payments and flexible plans that enable you to spread the cost of dental treatments over several months, it becomes feasible to accommodate your dental care needs within almost any budget. Feel free to inquire about how you can finance your dental care through CareCredit or Cherry Financing during your next appointment, and we will be happy to assist you through the application process.
